Discovery Call Scheduling – Once we receive your intake form, someone from our team will reach out to set up a discovery call, where we’ll explore if this retreat aligns with your personal goals.
This form is private and confidential and will only be viewed by your ceremony facilitator(s). Please answer all questions fully and honestly so that we are able to support you with safety and care. If there are any contraindications with the Sacred Earth Medicines we are working with, the facilitator will reach out to you directly.
By submitting this form, the applicant declares that they have read and understood the information in this medical form. They further declare that they have answered all the questions fully and honestly and have not withheld any information that they believe could be important. As far as they are aware, their general health is good.
As of the moment that they submit this health form, they commit to update the ceremony facilitator of any change in my physical or mental health status, of any inclusion or suspension of any medication (with or without medical prescription) and of the consumption of any drug or psychoactive substance (with the exception of tobacco, and cannabis).
